Refer a Patient To Our Clinic

In order to optimize health care resources, we go the extra mile to prevent duplicate investigations. When you refer a patient to us, please provide the following information to our staff.

Complete our Referral Request Form; Be sure to complete the “Reason for Referral” section in complete detail.

Once we receive the Referral Request Form, we will contact your staff and the patient to confirm a date for appointment and request additional information if necessary.

We may request previous investigations on a per case basis. Records may be faxed to our office at 770.466.1585. In the case of radiological studies, records and film may be brought to our office by the patient on the day of their visit.

  • CD copy of previous imaging studies such as CXR and Chest CT scan if applicable (Patients referred for lung nodule, lung mass, mediastinal or hilar lymphadenopathy, lung cancer, chronic cough, and shortness of breath or patients with a history of interstitial lung disease, pneumonia)
  • Copy of ECHO cardiogram report done over the past 12 months (Patients with symptoms of shortness of breath, a diagnosis or suspicion of pulmonary arterial hypertension or patients with cardiovascular disease)
  • Copy of Sleep Study results
  • Copy of any previous pulmonary function test or spirometry
  • Copy of any previous blood gas studies
  • Copy of six minute-walk test results
  • Copy of overnight oxymetry results
  • Copy of CBC and CMP or BMP
  • Copy of previous pulmonary consultation

  • Complete pulmonary function testing with state-of-the-art body plethysmography. Our equipment is capable of providing:
    • Spirometry pre and post bronchodilator
    • Lung volume pre and post bronchodilator
    • Airway resistance pre and post bronchodilator
    • DLCO
    • Overnight Oximetry
    • 6 minute Walk Test
    • Take Home Sleep Study (unattended)
    • In-Lab Sleep Study

Dr. Jeannot provides high quality pulmonary care, whether for inpatient procedures or outpatient procedures. This type of patient-focused care allows Dr. Jeannot to follow up with referred patients who are admitted to the hospital, ensuring better continuity of care.
